Dear Sir or Madam:
On my most recent statement for my Chase Platinum MasterCard, account xxxx-xxxx-xxxx-xxxx, there is a charge on July 5, 1998 for $30 from Fantastic Sam's, reference number WR0B. I had initially tried to pay for their services that day using my Chase Platinum MasterCard. However, the employee there had some trouble getting her machine to return an authorization, and then when an authorization was returned, no receipt was printed for me to sign. The employee asked to try a different credit card, and I gave her one of my other cards. Again, an authorization on that card was obtained, but no receipt was printed for me to sign. I decided that it was taking too long to try to pay by credit card, and I paid my bill in cash.
Please remove the $30 charge from my Chase Platinum MasterCard account, as I never signed a receipt authorizing that charge and have already paid cash for the services. I am enclosing a copy of my Chase statement as well as the statement from the other credit card showing the same charge on the same day.
